Add description text field to giving form

For some giving options it's helpful to let people know, what to do, and what comment to leave in the optional comment box. Please add a description field on top of the giving form we can customize. This is NOT the same as the comment field we already have! Donors can't change that field - it's for us to give then instructions, or even just a "thank you for giving to our church" text.

For contractors, it'd also be a place where one could list the job that the tithe was coming from for their records to make sure none was missed. Very helpful.

Yes! For example, if they select Giving Type "Missionary Support", we could add a note saying to put the missionary's name in the memo box, or even label the box "Missionary Name", and make it a required field.
